Opportunity Living is licensed to provide ICF-ID (Intermediate Care Facility for Intellectual Disabilities) level of care.  Opportunity living currently serves 68 individuals under this licensure.  These 68 individuals live in community based group homes that are located in Lake City and Rockwell City.  Opportunity Living provides work and activity services at its main location in Lake City.

 

Opportunity Living is also certified to provide HCBS (Home and Community Based Waiver Services).  We are able to serve individuals under the ID (Intellectual Disabilities) waiver as well as the BI (Brain Injury) waiver.

 

We are CARF (Commission on Accreditation of Rehabilitation Facility) certified which allows us to provide Pre-Vocational  services to waiver clients.

Opportunity Living awarded Three-Year CARF Accreditation
CARF International announced that Opportunity Living has been accredited for a period of three years for its Community Housing, Community Integration and Organizational Employment Services programs. The latest accreditation is the fourth consecutive Three-Year Accreditation that the international accrediting body, CARF, has awarded to Opportunity Living. 

This accreditation decision represents the highest level of accreditation that can be awarded to an organization and shows the organization’s substantial conformance to the CARF standards. An organization receiving a Three-Year Accreditation has put itself through a rigorous peer review process. It has demonstrated to a team of surveyors during an on-site visit its commitment to offering programs and services that are measurable, accountable, and of the highest quality. 

Opportunity Living is a nonprofit organization with offices at 1890 E. Main Street, Lake City, Iowa. It has been providing Intermediate Care for people with Intellectual Disabilities in Lake City and Rockwell City community based homes since 1989. 

CARF is an independent, nonprofit accrediting body whose mission is to promote the quality, value, and optimal outcomes of services through a consultative accreditation process that centers on enhancing the lives of the per-sons served. Founded in 1966 as the Commission on Accreditation of Rehabilitation Facilities, and now known as CARF International, the accrediting body establishes consumer-focused standards to help organizations measure and improve the quality of their programs and services.
